Payment Responsibility
Payment is due at the time services are rendered. Patients are financially responsible for all services provided, regardless of appointment type or length of visit.
Cancellation Policy
This office does not collect any fees for missed or late appointments. If you know you are going to be late, the office policy allows up to 10 minutes. The office of Beautifully Well understand life happens and you may need to cancel. There will be no penalty for being sick. However, if there are repeated appointment cancellations within a 48 hour time frame then future privileges may either result in pre-paying for future sessions or having all privileges terminated. If one no call/no show happens it’s an instant termination.
Insurance and Massage Therapy
Massage therapy services are not covered by insurance.
The only exception applies to VA patients with valid authorization, which may provide partial coverage. Any services beyond authorized coverage are the patient’s responsibility.
No other insurance providers cover massage therapy services.
Self-Pay Services
Patients receiving massage therapy services should expect to self-pay unless VA authorization is provided. Self-pay rates apply equally to all patients.
Billing and Collections
Outstanding balances may result in billing statements. Accounts that remain unpaid may be referred to a third-party collection agency if payment arrangements are not made.
Returned checks may be subject to a processing fee.
Equal Application of Policy
These Terms and Conditions apply equally to all patients. No exceptions are made based on insurance status, provider, appointment type, or duration of care.

Patients are encouraged to ask staff for clarification regarding these Terms and Conditions prior to receiving treatment.
Financial Acknowledgment
By accepting services, the patient acknowledges responsibility for all charges incurred, whether or not paid by insurance.
